Instructions for filling out the forms:

1. Make sure you have downloaded the latest version of Acrobat Reader
2. Complete the form using the TAB key to navigate throughout the form
3. IF YOUR COMPUTER WILL ALLOW Save a copy of the form to your own computer.
4. Send the file as an attachment to: blong@eccenter.com
5. IF YOUR COMPUTER WILL NOT ALLOW YOU TO SAVE A COPY OF THE COMPLETED FORM: Print a copy of the completed form and send it to the EC Church Center, Attn: Brenda, 100 West Park Avenue, Myerstown, PA  17067. ALSO print a copy for your own records
